Below are the insurance plans Roux Physical Therapy is currently contracted. We also work with various worker's comp companies and accept patients who are utilizing auto insurance benefits following a motor vehicle accident (MVA). 

This list is subject to change as we are credentialed with new insurances and experience network changes. We will verify your benefits when your first appointment is scheduled. 

If your insurance is not yet in network, please contact your insurance plan for out-of-pocket estimates. Roux Physical Therapy can provide documentation for patients who pay upfront for services and would like to submit claims to their out-of-network insurance providers.

Patients with out-of-network insurance providers are encouraged to contact their insurance company for estimates of out-of-pocket costs. Roux Physical Therapy can provide documentation for patients who pay upfront for services and would like to submit claims to their insurance providers who are currently not in-contract.

Yes, Roux Physical Therapy offers cash-pay dry-needling, recovery services and wellness products. Additionally, we will offer performance programs and/or monitored fitness options for patients who are discharged and would like to continue receiving services.

To ensure the privacy and confidentiality of our patients' medical records, medical record requests must be made in writing and signed by you or an authorized representative. Forms will be available in our office. A reasonable fee, in accordance with state regulations, may be charged to cover the cost of printing and mailing/shipping.
